I´ve got the same message and It tells me this (in VS Code Terminal) and I´m also trying to login again and It throws me an 500 Internal Server Error MCP client for `codex_apps` failed to start: MCP startup failed: handshaking with MCP server failed: Send message error Transport :StreamableHttpClientWorker>>] error: unexpected server response: HTTP 404: , when send initialize request
