I can't imagine how that would pass any kind of sniff test by Medicare.<p>Quote from the article: "In less than two years, more than $900 million in bogus claims were submitted to Medicare for grafts that were used on fewer than 500 patients, prosecutors said."<p>The alternative interpretation of this is that the grafts were _applied_ to 500 patients, but potentially many many more were billed for grafts that they didn't need (and didn't receive). Maybe more feasible?</p>

<p>Horrible, disgusting, yet not at all surprising. I don't know what medical billing accountability looks like in this dizzyingly complex system, but as a US citizen and patient of the US healthcare system, it looks barely existent. My family has fought our fair share of bogus healthcare charges. One instance: after my daughter was born at a birthing center (independent of a hospital), my wife experienced postpartum complications and was transferred to a nearby hospital for care. As if that experience wasn't stressful and traumatic enough, many months later we were hit with a surprise charge from the hospital for treatment and care of our newborn baby - the baby that was delivered hours earlier, in a separate location, that never left my arms in the hospital!<p>All that to say - it's alarmingly easy for a charge to get processed in a batch of other charges, and either insurance pays it without question, or the patient pays it unknowingly. During our experience, we learned that this kind of thing is exceedingly common. The power dynamic between patients and healthcare administration severely misaligned, the information imbalance is huge, and the patient is always in a compromised position.<p>The article doesn't include details on how they tracked down these criminals (I'm curious to know!), but it wouldn't surprise me in the least if this is just the tip of the iceberg.</p>

<p>I may be naive, but I'm not sure where the hate is coming from in this thread.  It's no secret that Google search result quality has rapidly and consistently diminished over the past few years, a common topic of discussion here on HN. Many folks, myself included, have developed a habit of appending "reddit" in Google searches by default. And reddit's native search, on the other hand, is just abysmal. If this partnership results in finding quality and timely content in reddit faster, then I'm all for it. Net positive IMO.</p>

<p>Content aside (which is both thought provoking and incredibly alarming) I’m fascinated by the author’s commitment to deeply understanding a system and the factors at play within that system. There’s something inspiring about relentless truth seeking, to the point of obsession, with some extraordinary level of meticulousness. Particularly when it challenges conventional wisdom. This is a slippery slope as it can quickly nosedive into junk science and contrarianism, despite best intentions, but when it’s done well, it’s really something.<p>Probably a faux pas here these days, but reminds me of Elon Musk’s First Principles.</p>

<p>I'm clearly naive and haven't worked closely enough to filtering stage of of the hiring process, but nevertheless.. I'm shocked to hear that there was a <i>deliberate</i> and explicit filtering by a list of "top schools. An implicit look at the school and some dose of bias, okay sure, I could see it, but an explicit filter?  Wow.</p>
