<p>In the project I've been working on in the past weeks with Claude Code, I have very clear instructions in the CLAUDE.md file. Things like:<p>- I'll push my work on branch X on this repo. Always fetch X to see if there is something new to merge<p>- Never touch files Y. These must be edited by hand, by me.<p>- If you make any changes to file Z, you must always explicitly ask for my confirmation.<p>It used to work very reliably. But for the past two days, Claude Code suddenly failed to follow these instructions several times, in different sessions, and right now even after the first prompt in a new session.
